Agency Referral Portal · North Carolina
Refer an older adult for housing — before the release date forces it.
A structured referral for reentry coordinators, parole and probation professionals, social workers, discharge planners and authorised community partners working with adults 60 and over in Raleigh and Wake County.

Thirteen groups refer to us, and each needs a different answer. Select yours.
Provider onboarding questions we ask
- • What must be verified before an address is approved?
- • Who holds approval authority, and what written evidence is required?
- • Documentation your process expects, and at what point.
- • Who obtains the person’s consent for a specific referral?
Prospective, not established
A referral conversation does not make your organisation a partner, and we do not describe any agency here as one. Partnership is only described once both sides have confirmed it in writing.
What happens when you submit
A referral is a conversation, not a placement.
Nothing is committed by sending this. Here is exactly what follows.
- 1
We acknowledge receipt and record the referral with a named reviewer.
- 2
We confirm the housing requirements with you — area, budget, timeline, supervision.
- 3
We tell you what we can and cannot meet. If we have no suitable property, we say so early.
- 4
Where a property fits, we confirm availability and share its real facts.
- 5
Address approval, if required, goes through your authorised process — not us.
- 6
Written terms, payment confirmation and move-in details are completed last.
What this form must not contain
No Social Security numbers, medical records, medication lists, detailed criminal histories or benefits documentation. This is an ordinary web form, not a secure clinical portal. Referral, housing acceptance and payment authorisation remain three separate steps.
Working with a family?
Send them to the family referral route, which does not require case details and keeps their inquiry in a separate queue from agency referrals.
