For Agencies

You see the housing need every week. We want to hear about it.

We welcome conversations with discharge planners, social workers, case managers, reentry teams and community organizations about housing needs, referral requirements and partnership opportunities.

Photography on this page is illustrative. It does not depict PHS staff, residents or partners.

Housing Demand Intelligence Desk

Tell us who you’re having trouble housing.

We build housing around documented demand — which populations, which counties, what budgets and what approvals. Nine questions, about five minutes, and no client information required.

A conversation, not a placement request

An agency housing inquiry documents what your community needs and how a partnership could work. It does not authorize payment and does not by itself confirm a placement. Referrals, housing acceptance and payment authorization are three separate steps.

Two directions, two pathways

Most of this page is for organizations seeking housing for the people they serve. If you are an existing housing provider seeking residents, your pathway runs the other direction and is grouped separately below.

Providers seeking residents

The other direction.

If you already operate independent housing and have capacity to fill, we want to understand your model. Different questions, different pathway — we will not answer a provider conversation with an agency process.

Existing Housing Providers Seeking Residents

For operators with rooms to fill and a housing mission.

Not sure which you are?

If you place people into housing, you are seeking housing. If you have rooms and need residents, you are seeking residents. If you do both — which some organizations do — start with whichever conversation is more urgent and tell us about the other in the notes.

How we work with agencies

Six things, in order.

This is the sequence behind every agency partnership. It is documented rather than improvised, and each step has an output.

01

Requirements discovery

We start with your process, not our assumptions — what you must confirm, what documentation you need and who is authorized to provide it.

02

Geographic needs assessment

Which counties your clients need to live in, and where placements consistently stall. Gaps are the most useful thing you can tell us.

03

Housing and property partnership development

We work with property owners on ownership authority, proposed use, local demand, condition, access conditions, insurance, costs and a reviewed written arrangement.

04

Clear contact ownership

A named owner with a backup on every record, so your inquiry does not depend on one person being available.

05

Documented follow-up

A next action and a due date on the record. If it is going to slip, you hear that from us rather than discovering it.

06

Coordination with outside providers

Clinical care, case management and personal care stay with your team or the resident's existing providers. We say so plainly and build around it.

Four ways to start

Choose the lightest option that fits.

  • Discuss Your Housing Needs

A structured conversation about counties, demand, formats, affordability and referral requirements.

    Start the conversation
  • Request an Agency Introduction

A short briefing for your team on what PHS does, how we work and what a partnership involves.

    Request a briefing
  • Help Shape Housing in Your County

If your county is not a current focus area, tell us what your community needs and help set our priorities.

    Bring PHS to your county
  • Download Our Partnership Overview

Who we are, what we do and how a partnership conversation starts — including our current markets.

Fill this in before the call

The Agency Housing-Needs Worksheet uses the same structure we do. Bringing it complete turns a first call into a working session.

Open the worksheet
  • Do not include client names, DOBs, SSNs or diagnoses
  • Aggregate need only — no individual cases
  • Ranges are more useful than invented precision

Independent, nonmedical housing

This housing model does not include personal care, clinical treatment, medication management or supervision. Outside supports are arranged independently where appropriate. Accommodation requests receive individualized review — we do not apply blanket exclusions based on disability or the use of assistive devices.

What we do not promise

We do not promise placements, clinical outcomes, government payments or guaranteed referrals. What we do promise is a documented process, a named owner on your inquiry and an honest answer.

Bring us the gap you cannot fill.

The most valuable thing an agency can tell us is where placements consistently fail. That is what we will work toward first.
