Agency Housing-Needs Worksheet

Version 2.0 · Approved 2026-10-10 · Owner: Partnerships · Reviewed quarterly

How to use this worksheet

Fill this in internally first. It is the same structure we use in a housing-needs conversation, so bringing it complete makes the first call significantly more useful.

This worksheet captures aggregate need. Do not include client names, dates of birth, SSNs, diagnoses, medical records, criminal histories or financial documents.

1 — Geography

Which counties do the people you serve most often need to live in?

Are there counties where you consistently cannot find a placement? Those gaps are the most useful thing you can tell us.

2 — Household and room format

Typical household size (1 person, 2 people, larger).

Room format that works: private room in a shared home, shared adult-household room, or other.

3 — Affordability and payer category

Approximate affordability range.

Payer category you typically work with: benefits income, authorized housing assistance, employer or workforce program, self-pay, or not yet determined.

Note any process your program requires for authorized housing assistance to be approved.

4 — Referral requirements

What must your team confirm before a housing destination is approved?

What documentation does your process expect, and who is authorized to provide it?

How much advance notice do you typically have?

5 — Outside supports

Which support services will be arranged separately from housing (clinical, personal care, case management, behavioural health)?

Who arranges and funds them? Independent housing does not include these services.

6 — Volume and timing

How many placements do you realistically need in the next 6 and 12 months?

When would you want a housing partner ready?

About this document

This is an original Preferred Housing Solutions document describing our own approach and process. It is not an executed contract, and it is not an approved agency, government or funder document. Nothing in it reserves housing, confirms a placement or authorizes payment.

Ready to use this in a real conversation?

An agency housing inquiry or a property partnership inquiry is where these documents do their actual work.
