Connect With Us
Six ways to start a conversation — pick the one that fits.
Each route goes to a different pipeline with different questions and a different reviewer. Choosing the closest one gets you a more useful first response.
Every inquiry gets an owner, a next action and a date
Nothing you submit here reserves housing
Every form on this site is a partnership or information inquiry. None of them reserves housing, confirms a placement, approves an application or authorizes payment. Please do not submit client names, dates of birth, SSNs, diagnoses, medical records, criminal histories or financial documents through any public form.
Which route do I need?
Six routes, described plainly.
If two seem to apply, choose the closest and say so in the notes. We will route it to the right person either way.
Agency housing-needs conversation
You represent an agency with housing need. Tell us counties, demand, formats, affordability and referral requirements.
Who it is for: Agencies and housing teams
Discuss your housing needsProperty partnership inquiry
You own or represent a property. Tell us the county, type, availability, asking rent and your authority to discuss it.
Who it is for: Property owners and representatives
Start a property inquiryAgency introduction / briefing request
Request a short briefing for your team on what PHS is developing, what we do not provide, and how our process works.
Who it is for: Agency teams and leadership
Request a briefingFuture housing updates
Ask to be told when a property in your region is verified and ready. We will not send availability claims before then.
Who it is for: Agencies, property owners, community partners
Sign up for updatesGeneral questions
Something that does not fit the other categories. We answer approved public information and route everything else.
Who it is for: Everyone
Send a questionAccommodation / contact concerns
An accommodation request, or a concern about how you have been treated. Concerns about our own conduct are a separate track.
Who it is for: Anyone in contact with PHS
Raise an accommodation or concernWhat happens after you submit
A named owner, a reference number, and a next-update time.
- 1
Confirmation only after receipt
Success shows after your submission is confirmed received — never optimistically. If delivery fails, our staff are alerted rather than the inquiry being lost.
- 2
Assigned to a named owner with a backup
Both pipelines — agency and property — assign an owner and a backup at intake, so an absence does not stall your inquiry.
- 3
A reference number and a next-update time
You receive a reference prefix identifying the workflow, and a specific next-update window rather than an open-ended promise.
- 4
Requirements documented, then follow-up
Discovery scheduled, requirements documented, then follow-up. You always know which stage your inquiry is at.
Our pipelines, in full
Separate pipelines run for agency and property-partner inquiries. Both follow the same stages.
- 1New Inquiry
- 2Assigned
- 3Contact Attempted
- 4Discovery Scheduled
- 5Requirements Documented
- 6Follow-Up

Direct phone, email and staffed hours are published once verified. Until then, use the forms above — your submission is routed to the team, and you receive a reference number on receipt.
Concerns about our conduct
If you believe PHS or a partner has acted improperly, use the accommodation and concerns route. Concerns about our own conduct are handled on a separate track from accommodation requests, so that neither dilutes the other.
Raise a concernDonations are not part of this process
Partnership inquiries and donations are kept entirely separate. We are not soliciting donations on this site, and no tax-deductibility or fundraising claim is made anywhere on it until the applicable status and requirements are verified.
AI assistance, limited on purpose
Any automated assistance on this site may explain approved public information, answer general questions and direct you to the correct inquiry form. It does not approve applicants, determine eligibility, give clinical advice, promise funding, disclose private records, or claim housing is available. Those decisions are made by people, and only after verification.
Direct phone and staffed hours are published once verified. In the meantime, use the inquiry forms and your submission is routed to our partnership team.
Still not sure which route fits?
Send a general question and describe your situation in a sentence or two. We will point you to the right form rather than making you guess.
