What this tool does — and does not do
This is an organisational tool. It asks informational questions and produces a summary. It does not assess anyone for housing, score anyone, determine eligibility, diagnose anything or make any decision about a person. It does not submit anything — you choose what to do with the result. Every summary routes to a human conversation.
List the counties, and flag any where you consistently cannot find a placement. Those gaps are the most useful thing you can tell us.
Please do not enter client names, dates of birth, SSNs, diagnoses, medical records, criminal histories or financial documents. This planner is for aggregate organisational need.
